What is the National Disability Insurance Scheme (NDIS) and the role of Support Coordination?

The NDIS is a new way to give choice and control to people with a disability. You the participant can chose which service provider you would like to use and how the supports are given. Support Coordination is an important part of the funded support in your plan. Implementing your NDIS plan is the first step in achieving your goals.

What is Support Coordination?

The objective of Support Coordination is to connect you to appropriate NDIS supports and service in your area. At Huntington’s Australia we will provide you with a highly skilled Support Coordinator to assist you to implement all funded supports in ‘YOUR NDIS PLAN’ including all informal, mainstream and community supports in a way that is directed by you the participant. Essentially, we will help you navigate the NDIS.

The NDIS states that Support Coordination is:

  • A capacity building support, funded in the NDIS plan
  •  To assist people with a disability and their family and carers to navigate the NDIS system
  •  These include funded supports, informal, government and community supports
What is Specialist Support Coordination?

Specialist support coordination is a higher level of support. The focus is on reducing complexity in the participant’s support environment and helping the participant overcome immediate and/or significant barriers in plan implementation. At Huntington’s Australia we will provide you with a highly skilled Specialist Support Coordinator to assist you to address complex barriers that affect your access to support and design a service plan that supports your needs.

Our Support Coordinators will:

  •  Assist you to access community activities, support with health, informal supports and funded supports
  •  Work in a person-centred approach, putting you first
  •  Help you chose the services that will suit your needs
  •  Assist you to negotiate with providers to maximise the best out of your plan
  •  Make sure service agreements and providers service bookings are created
  •  Work with you to achieve your goals
  •  Organise appropriate assessments e.g. Occupational Therapy (OT) capacity assessment or home modification
  •  Complete annual reports for the next planning meeting
  •  Support you to resolve and issues or disputes with service providers
  •  Assist with suitable housing options, respite, supportive accommodation
  •  Guide you through what the NDIS means by reasonable and necessary

We are accredited to deliver both level 2 and level 3 Support Coordination.

This support item strengthens a participant’s ability to design and then build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment. Coordination of Supports is to focus on supporting participants to direct their lives, not just their services, and is focussed on assisting participants to build and maintain a resilient network of formal and informal supports. This involves working together with the participant to understand the funding, identify what participants expect from services, and how participants want this designed. Coordination of Supports also includes coaching participants and working with participants to develop capacity and resilience in their network.

This support is delivered utilising an expert or specialist approach, necessitated by specific high complex needs or high-level risks in a participant’s situation. Specialist Support Coordination is delivered by an appropriately qualified and experienced practitioner to meet the individual needs of the participant’s circumstances such as a Psychologist, Occupational Therapist, Social Worker, or Mental Health Nurse. Specialist Support Coordination is expected to address complex barriers impacting a participant’s ability to implement their plan and access appropriate supports. Specialist Support Coordinators assist participants to reduce complexity in their support environment and overcome barriers to connecting with broader systems of supports as well as funded supports.

Specialist Support Coordinators are expected to negotiate appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation. Specialist Support Coordinators will assist stakeholders with resolving points of crisis for participants, assist to ensure a consistent delivery of service and access to relevant supports during crisis situations.

Specialist Support Coordination is generally delivered through an intensive and time limited period necessitated by the participant’s immediate and significant barriers to plan implementation. Depending on individual circumstances, a Specialist Support Coordinator may also design a complex service plan that focusses on how all the stakeholders in a participant’s life will interact to resolve barriers and promote appropriate plan implementation. Once developed, a Specialist Support Coordinator will continue to monitor the plan, but it may be maintained by one of the participant’s support workers or other care support.
